GENERAL SUMMARY:

The BHC Population Health Quality Improvement Advisor works with the Clinical Integration and Population Health Manager to provide consultative services to Bryan Health Connect (BHC) member clinics in managing the overall health of populations. The purpose of this position is to work collaboratively with member clinics to improve quality and decrease costs through assessment of clinical performance, identification of gaps in best practice, and recommend improvement methods to transform patient care processes and improve quality outcomes.
   
PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.
2. *Works with the Chief Medical Officer and the Clinical Integration and Population Health Manager to implement and maintain the population health management strategy to develop and execute work plans to engage physicians, providers and community partners to improve healthcare quality and consumer engagement.
3. *Travels extensively between sites approximately 3-5 days per week, including evening and early morning meetings as needed. Travel is primarily during the business day, but some overnight travel may be required.
4. *Serves as a consultant in clinical practice transformation activities including:
 a) Establishing relationships and acts as a trusted resource for BHC providers and clinic personnel to advance the clinical priorities of BHC.
 b) Working closely with an identified group of practices to develop and deploy work plans that will help their clinics achieve success.
 c) Serving as a mentor and coach for practice level improvement teams that refine practice and population management workflows.
 d) Meeting at least quarterly with assigned clinics to discuss performance reports and BHC clinical priorities.
5. *Ensures Quality Improvement initiatives are prioritized throughout BHC by:
 a) Utilizing innovative nursing and population management tactics to support health throughout the continuum of care in order to promote quality, cost-effective outcomes.
 b) Facilitating quality improvement activities that educate, support, and monitor provider(s), regarding implementation of evidence-based care for best practice.
 c) Creating and distributing education to clinic staff and providers based on clinical priorities.
6. *Utilizes Data Analytics to focus on areas in need of improvement and evaluation of continuous quality improvement by:
 a) Working with data and analytics team to produce actionable reports and identify opportunities for targeted, scalable interventions.
 b) Utilizing data and evidence-based research to identify practice variability and define targeted and scalable initiatives for improvement as directed by the BHC Clinical Guidelines/Quality Assurance Committee and BHC leadership.
 c) Presenting complex data and work effectively with physicians, administrators and other healthcare professionals.
7. Performs other related duties and projects as assigned.

EDUCATION AND EXPERIENCE:

Bachelor’s degree in Nursing preferred.  Minimum five (5) years relevant healthcare experience preferably in an ambulatory care setting, care management, case management or clinical coordination.

Experience in practice re-design work, practice transformation, quality improvement, population health management, ambulatory care setting, quality and efficiency metrics, and using data to set goals, influence stakeholders and measure progress are highly desired. Previous experience in electronic health/medical records preferred. Experience in performance improvement and project management preferred.
 
OTHER CREDENTIALS / CERTIFICATIONS:

Valid State of Nebraska driver’s license required.  Verification and maintenance of responsible driving record per facility standards required.
