GENERAL SUMMARY:

Directs the discharge planners within their roles of Care Management. Plans successful discharge plans for patients and families. Focuses on setting up patients and their families for safe and successful outcomes and decreasing re-admissions. Assists clients by assessing their situation, setting goals, and obtaining required services. Aides in determining the nature of specific client’s situation by assessing medical, psychological, emotional, and social information. Works with the Care Management team and other members of the interdisciplinary team.

 

PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Maintains regulatory requirements including all state, federal and accreditation regulations.

3. *Supports patients in making informed decisions by acting as their advocate regarding patient clinical status and treatment options.

4. *Performs a psychosocial assessment and reassessment on all patients. Assesses and addresses motivational and psychosocial issues. This includes obstetric, pediatric, adolescent, geriatric, and the general patient population.

5. Maintains open communication interdepartmentally and interdisciplinary.

6. *Ensures that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels.

7. *Handles case assignments, reviews concurrent case progress and can adjusts plans accordingly.

8. *Records and documents case information, accurately completing all necessary forms and producing statistical reports.

9. Interacts professionally with patient/family and involves them in forming the plan of care.

10. Gains knowledge of patient’s financial status and diagnosis to better assist with patient’s discharge needs.

11. *Responsible for home care needs being met by the time of discharge, with a goal of arrangements completed 24 hours prior to discharge when date of discharge is known.

12. *Prepares patients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post discharge requirements; orienting and training family members; providing resources.

13. *Performs and evaluates mental health, drug, and ETOH assessments.

14. *Provides immediate counseling/support of patients and family, effectively diffuses crisis and refers to appropriate services for abuse/neglect as needed, such as APS, CPS.

15. *Facilitates ongoing communication with Child Protective Services (CPS), law enforcement and county attorney's office.

16. Under provisional supervision, provides crisis/supportive counseling and coaching to patients. This includes a treatment plan for short-term and long-term goals for the patient.

17. Provides supervision and education to bachelor-level social workers within the case management department.

18. Provides supervision to bachelor-level social work students completing practicum.

19. Provides education to patients and providers regarding insurance coverage, including Medicaid eligibility.

20. Facilitates patient engagement and follow-up care by providing education and support to patients/families regarding chronic conditions.

21. *Strengthens community relationships.

22. *Tracks and trends delay in Care Coordination and presents results to Utilization Review Committee.

23. Assists with scheduling the Care Coordination department.

24. Leads and assists with problem solving within the Care Coordination department, outside facilities, and the interdisciplinary team.

25. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

26. Participates in meetings, committees and department projects as assigned.

27. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

1. Maintains clinical competency as required for the unit including but not limited to age-specific competencies relative to patient’s growth and developmental needs, annual skill competency verification and mandatory education and competencies.

2. Knowledge of management and leadership principles and practices.

3. Knowledge of criteria for Medicare, Medicaid, HMO, and private insurance coverage.

4. Knowledge of evidence-based counseling techniques to the degree necessary to provide appropriate emotional support to the patients and their families. This includes but is not limited to motivational interviewing and behavioral activation.

5. Knowledge of criteria for Medicare, Medicaid, HMO, and private insurance coverage.

6. Knowledge of computer hardware equipment and software applications relevant to work functions.

7. Ability to submit and interpret Pre-admission Screening and Resident Review (PASRR).

8. Ability to communicate effectively both verbally and in writing.

9. Ability to establish and maintain effective working relationships with all levels of personnel and medical staff.

10. Ability to problem solve and engage independent critical thinking skills.

11. Ability to deal with resistance and complex situations.

12. Ability to maintain confidentiality relevant to sensitive information.

13. Ability to prioritize work demands and work with minimal supervision.

14. Ability to maintain regular and punctual attendance.

 

EDUCATION AND EXPERIENCE:

Bachelor’s degree in social work required. Current license as a Social Worker in the State of Nebraska required. Master’s degree in social work from accredited university preferred. Five (5) years of medical social work experience preferred.

 

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
