GENERAL SUMMARY:
Responsible for the provision and oversight of professional nursing care and medical care prescribed by the Licensed Healthcare Provider (Physician, APRN, or PA). Supports a provider in a primary role, coordinating and facilitating patient care and follow up. Assists provider with examinations and procedures to serve the medical needs of patients with various cardiac and vascular related health conditions in a professional, confidential, and caring manner. Provides care that is beyond the practice scope of LPN II and CMA II assigned to support a provider in a primary role; including triage of patient follow up, patient teaching, and abnormal test result communication. Accountable for having knowledge of and complying with the Nebraska Nurse Practice Act and other relevant State Nurse Practice Acts, any applicable licensure/certification requirements, as well as, organizational policies and procedures.
PRINCIPAL JOB FUNCTIONS:
1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values in interactions with providers, staff, patients, and families.
2. *Supports provider(s) in a primary role, coordinating and facilitating patient care and follow up to achieve quality patient care and customer service goals.
3. *Performs nursing care that requires professional knowledge and judgement for assigned providers, including triage of patient follow up, patient teaching and abnormal test result communication.
4. * Retrieves test results, as soon as available, communicates with the provider and contacts the patient regarding abnormal results; may delegate communication of normal results.
5. *Monitors patient caseload for high-risk patients, compliance with physician orders and follow up appointments and referrals.
6. *Triages and responds to calls from patients, pharmacies, hospitals, nursing homes and other medical offices by use of telephone and computer, in a timely manner. Responds appropriately to urgent/emergent situations, documents as necessary and manages transfer of care.
7. *Prepares Electronic Health Record (EHR) for patient visit, ensuring all necessary information is in the chart prior to appointment, as well as, documenting patient interactions, orders, results, patient questions/concerns and medication changes, in a timely manner.
8. *Facilitates flow of patients in accordance with appointment time to avoid delays for the provider and undue waiting for patients.
9. *Conducts new patient history and assessment, reviews established patient history updates; including pain assessment, domestic abuse review, medication reconciliation and allergy review.
10. *Educates patient and family about tests/procedures, results, medications, nutrition and other health related issues.
11. *Assists with rooming; including vital signs, patient history, preparing patient for examination/procedures, performing EKG, as well as, keeping rooms stocked, clean and ready for use.
12. *Rooms patients via EHR status indicator for outstate clinics and enters clinical documentation, orders, and results received from clinic, as assigned.
13. *Complies with HIPAA and maintains confidentiality in matters related to patient, family, staff, and providers.
14. * Provides care in a non-judgmental, compassionate, non-discriminatory manner remaining sensitive to the patient’s diversity, autonomy, dignity, and rights.
15. *Assists patients with pre-authorizations, release of information requests, FMLA certification, etc., as needed.
16. *Responds appropriately to urgent /emergent situations, documents as necessary and manages transfer of care.
17. *Maintains compliance with OSHA and safety plan.
18. Notifies Team Leader of problems/incidents related to staff or patients in a timely manner.
19. Participates in meetings related to performance improvement, electronic health record enhancements, and day-to-day operations, as assigned.
20. Manages work hours, supplies, and other clinical costs in line with fiscal responsibility and budget.
21. Assists provider(s) in both Lincoln and Outreach Clinic sites, as assigned.
22. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
23. Participates in meetings, committees and department projects as assigned.
24. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
1. Maintains clinical competency as required for the unit including but not limited to age-specific competencies relative to patient’s growth and developmental needs, annual skill competency verification and mandatory education and competencies.
2. Knowledge of nursing, clinical systems and the assigned medical specialty.
3. Knowledge of diagnosis allowed for ordering tests/procedures to support appropriate orders and avoid unnecessary denials and delays in care.
4. Knowledge of federal and state regulations related to nursing practice, OSHA and HIPAA requirements.
5. Knowledge of computer hardware equipment and software applications relevant to work functions.

EDUCATION AND EXPERIENCE:
Graduation from an accredited Registered Nursing Program is required. BSN is preferred. Current licensure as an RN in the State of Nebraska is required. Minimum of one (1) year relevant nursing experience is required. Clinic and cardiovascular nursing experience is preferred.
Licensure as an RN in the State of Kansas is required for nurses traveling to Kansas clinics and in other states where the nurse practices in the clinic, and/or patients, who receive care or instruction by phone, reside.
